Being a Care Coordinator is about more than just rota management. You will be responsible for supporting the Manager with Business Growth and ensuring we are always delivering care without compromise to our clients.

We pride ourselves on delivering care that truly meets the needs of the individual we support, and your role will be fundamental to ensuring this happens every day by matching Customers and Carers not only based on availability, but also personality.

Responsibilities of the role: (But not limited to)

  • Knowledgeable with PASS For Care System and CarePlanner System is an added advantage.

  • Arranging initial visits to, and the assessment of, prospective Service Users to decide whether Jewel Home Care Ltd is able to meet their personal care needs.

  • Conduct Risk Assessments as part of the Individual Needs Assessment and to update these as required as part of the Service User’s risk management.

  • Delivering personal and non-personal care to customers if necessary

  • Carry out introductory visits every time a Care Worker visits a Service User for the first time.

  • To be on-call in case of emergency;

  • Liaise closely with Care Coordinators regarding recruitment, permanent allocation of Care Workers and the writing of Care & Support Plans

  • Identify any training needs of Care Workers and report accordingly;

  • Conduct Care Reviews and respond to Individual Review Forms completed by Care Workers

  • Report any changes in a Service User to the Local Authorities

  • Monitor, supervise and mentor all Care Workers in the field

  • Encourage and identify opportunities for Care Workers’ continuing professional development.

  • Monitor all Service Users for customer satisfaction (Reviews)

  • Ensure the accuracy and completeness of all documentation held in Service Users care Plan

  • Ensure that all written reports and administration is up to date

  • Liaise closely with the Manager and other teams in the office and report back regularly

· Able to work as part of a team as well as under own initiative

· Skilled logistics

· Highly organised and able to prioritise.

· Good oral and written communication skills

· Knowledge of domiciliary care provision.

Preferable:

·Have or be willing to work towards a QCF (NVQ) level 3 in Care/ Management or equivalent.

· Domiciliary Care experience.

Required:

· IT skills

· Excellent customer service skills

Personal Qualities:

· Pleasant, helpful, good concise telephone manner with customer focus

· Flexibility to cope with changing demands and priorities

· Calm under pressure

· Own transport essential

· A strong focus on teamwork
