Care Co-Ordinator

Care Coordinator Chertsey/Woking Remedicare Staffing is recruiting for a new Care Coordinator for a new role based in Chertsey. Salary for the Care Coordinator Role : £28,000 – £30,000 per year Hours : Monday to Friday (On call) Duties for the Care Coordinator Role: * Make sure there is sufficient cover to deliver care and support on time * Use IT systems to allocate care assistants to provide care and support to customers. * Travel arrangements, routes and working patterns to make efficient use of care assistant time whilst meeting the needs of customers * Changes in care provision and support * Priorities when there are unexpected emergencies * Distribute staff rotas timely manner * Take part in the out of hours emergency on-call rota * Work with the office team to maintain up to date electronic and hand written records * Work with the team and other health and social care professionals to deliver high quality homecare services and to make improvements where necessary * Work flexibly as part of the team and apply policies and procedures at all times Requirements for the Care Coordinator Role: * Good administrative and IT skills. High level of accuracy and attention to detail * Ability to communicate clearly and build positive working relationships with customers, their families or representatives, staff and other health and social care professionals. * Good telephone manner and interpersonal skills * Good understanding of the needs of people who need care and support * Sound knowledge of the local area * Ability and willingness to follow policies, procedures and instructions and to participate in training * Minimum of 2 years’ experience as a Care Coordinator If you are interested in this role please feel free to give me a call or drop me an email
