Responsibilities:
- Conduct assessments and develop personalised care plans to meet each patient\’s unique needs.
- Serve as a support system for patients and families, providing ongoing care coordination and emotional support.
- Complete holistic needs assessments that address physical, emotional, and social aspects of care.
- Educate and mentor staff and learners, promoting best practices in head and neck cancer care.
- Conduct comprehensive Holistic Needs Assessments at key points along the cancer pathway.
- Assess, plan, implement, and evaluate nursing interventions to meet patients\’ needs.
- Collaborate with the multidisciplinary team (MDT) and attend meetings to ensure patients\’ wishes are prioritised.
- Under the guidance of the Head and Neck Clinical Nurse Specialist, provide specialised nursing care and support to patients with Head, Neck, and cancer, along with their families/carers.
- Support patients across various care settings, including wards and outpatient clinics.
- Act as an advocate for patients, families, and carers when needed.
- Assist patients in achieving self-care and educate caregivers to support them at home or in the hospital.
- Empower patients to manage their health and well-being, ensuring they can express their preferences, worries, and concerns.
