Leaving Care Personal Advisor

Are you passionate about making a lasting difference in the lives of young people? Do you have the drive to support and empower care leavers as they transition into adulthood? If so, we want to hear from you.

We're looking for a Personal Advisor (PA) to join the dedicated Leaving Care Team in Tameside, supporting young people aged 16+ as they navigate the journey into independent living. Many of the young people we support have experienced trauma and adversity, and your role will be instrumental in building trust, promoting stability, and helping them thrive.

What You’ll Do:

• Build strong, supportive relationships with care leavers to help them feel connected, safe, and valued.
• Co-produce tailored pathway plans that reflect the young person’s wishes, needs, and aspirations.
• Advocate for young people, working in partnership with a range of services including housing, education, health, and training providers.
• Carry out regular visits, direct work, and assessments in line with statutory guidance.
• Support young people from diverse backgrounds, respecting their cultural, religious, and personal identities.
• Promote trauma-informed, anti-discriminatory, and strengths-based approaches to practice.
• Participate in duty systems, supervision, training, and team development activities.
