Head of Community Palliative Care East Team

The postholder will work collaboratively with the Head of Community Services West in developing and managing an integrated, multidisciplinary community service, providing specialist palliative care.

Main Duties of the Job

The Role

You will provide leadership and direction to our integrated community services who provide holistic, skilled, high-quality, patient-centred palliative care to patients in their own homes, in partnership with other organizations and in appropriate sites across our boroughs. Care is delivered at home or in a care home, clinics, community spaces or our patient’s usual place of residence.

You will support the design, and lead delivery of, our clinical strategy, while providing a sense of purpose to your service. One of our key aims is to increase clinic and group work across our community, reaching those who are least likely to access our services and identifying potential partnership opportunities within the boroughs to provide care closer to home. This is where your outstanding communication skills will be essential in engaging diverse communities from across our local area.

Working as part of the Patient Services Directorate, you will support the Director of Patient Services in implementing and reporting against achievements in our strategic plan with a view to developing an inspiring culture of excellence across the integrated community team. You will be key to delivering our Clinical Strategy that will enable us to reach more people and ensure our services are delivered in line with our CORE values across our integrated community patient services.

Detailed Job Description and Main Responsibilities

We are looking for a nurse leader who has vision and creativity, someone who can think on their feet and problem solve when needed, but also work strategically and ensure that the ‘bigger picture’ is considered when making decisions. We require someone who can deliver on targets whilst also running services in new and innovative ways.

The successful candidate will have outstanding communication skills and be able to engage with people across our community. We are looking for someone who is just as comfortable communicating with a patient as they are with a volunteer or Chief Executive.

You will be part of a clinical leadership team as the Head of Community Palliative Care East Team on a permanent, full-time basis.
