———————————————————————————————————————————————————————————————-, To provide a Structured Clinical Management in line with the Trust pathway and standard operating procedure, to provide specialist personality disorder assessments, to facilitate SCM groups and provide SCM supervisions, attend weekly supervision and provide/attend training as required.

The post holder will demonstrate an attitude which respects and values service users’ and their carers., Undertake personality disorder assessments and develop Living Life Well Crisis Care Plans

To maintain a case load of 12 SCM clients (for assessment, socialisation or full programme) and also co-facilitate a SCM group – if the post holder is not running a group then to maintain a case load of 15 clients.

To attend weekly SCM supervision meetings

To attend training programmes (as when required)

To support and provide training to other staff

To be active in managing the clients pathway through the system, including in-reach work on the ward.

During admissions to liaise with the ward staff and Home Treatment Team and remain in contact with the service user throughout involvement with these service. Try to ensure the admission has clear goals, aims and objectives and take a lead role in supporting discharge/transition.

Facilitate a planned discharge. Organise an MDT meeting if necessary. Provide information regarding how to re-access services.

To advocate for clients with a personality disorder.

To implement the SCM treatment programme for borderline personality disorder

Co-facilitate weekly SCM group meetings (90 minutes).

Named practitioner offering a transition period from services for 6 months for clients who have completed 12 months of SCM

To work in accordance with the standard operating procedure for cluster 8

Formulate and implement appropriate treatment plans and provide Structured Clinical Management

Provide short term goal focused support / input to cases on the pathway

Liaise with other professionals to contribute to the establishment of continuity of care across the service

To ensure that continuity of care is maintained and to support services to manage transitions between services in line with NICE guidance, including Section 117

To carry out assessments as per Patient Owned Database and assign patients to care clusters within the framework of the Health of the Nation Outcome Scales Payment by Results (HoNOS and PBR)

To be able to provide consultation and training in personality disorder to other team members as required

Demonstrate an ability to formulate clients mental health needs

Agree to undertake additional training when required to facilitate role

To support the adherence of the personality disorder pathway.
