Tower Hamlets Council is seeking an experienced and dynamic Social Worker to join the Hospital Social Work Team. This is a fast-paced role focusing on safe and timely hospital discharges, working under the Discharge to Assess (D2A) Model – Home First Policy.

Key Responsibilities
? Hospital Discharge Planning
Carry out assessments for hospital patients to ensure safe and timely discharge.
Implement Discharge to Assess (D2A) and Home First policies.
Work closely with hospitals, community services, and external agencies.
? Fast-Paced, Short-Term Caseloads
Manage a quick turnover of cases – no waiting list.
Ensure seamless case transfer to Locality Teams post-discharge.
? Multi-Agency Collaboration
Work in partnership with NHS, social care teams, therapists, and voluntary sector organisations.
Attend multi-disciplinary team meetings to support effective discharge planning.
? Assessment & Safeguarding
Undertake Care Act 2014 assessments, Mental Capacity Assessments (MCA), and Best Interest Decisions (BID).
Identify safeguarding concerns and complete investigations where necessary.
? Hybrid Working & Professional Development
? Qualified Social Worker registered with Social Work England
? Proven experience in hospital discharge, D2A, and multi-agency working
? Knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding procedures
? Strong assessment and crisis intervention skills
? Hybrid working flexibility – able to work on-site and remotely as required
? Immediate availability preferred
