We are looking for an Occupational Therapist to –

  • Completing complex moving and handling

  • Functional assessments – Completing reablement plans and liaising with care agencies

  • Reduced care reviews

  • DFG referrals for major adaptations

  • Minor works referrals and equipment provision

Essential Requirements : HCPC registered , 2 years Social Services Experience , Car driver
