You will work as part of a close-knit team who are dedicated to the health and wellbeing of people in Gravesend. Gravesend is made up of a diverse patient group with complex needs., * To facilitate the assessment and management of children and young people referred for assessment: ensuring the quality of care provision using Trust policies and guidelines and care pathways.

  • To support diagnostic assessments undertaken by medical staff

  • To assess for clinical prioritisation and create robust plans of care

  • To regularly evaluate the service provided for children/young people whose lives are affected by neuro-developmental disorders, advising of changes to the delivery of care if they are needed.

  • To provide support, education and advice to children with neuro-developmental disorders and their families, carers, health and education and other agencies, CAMHS, Social Services andother professionals.

  • To provide and integrated service working closely with the paediatricians and other neuro-developmental nurses and liaising with CAMHS in selected cases.
