Community Mental Health Nurse

As a Community Psychiatric Nurse, you will be responsible for providing comprehensive mental health care and support to clients in their homes or community settings. You will work closely with a multidisciplinary team to ensure the delivery of holistic and person-centered care., * Conduct comprehensive mental health assessments and develop individualized care plans.

• Provide therapeutic interventions, including medication management and psychological support.

• Monitor and review clients’ progress, adjusting care plans as necessary.

• Offer crisis intervention and support to clients and their families.

• Collaborate with other healthcare professionals, including GPs, social workers, and psychologists.

• Maintain accurate and up-to-date clinical records in accordance with professional standards and guidelines.

• Promote mental health awareness and education within the community.

• Participate in multidisciplinary team meetings and case conferences.

• Registered Mental Health Nurse (RMN) with a valid NMC pin.

• Previous experience working as a Community Psychiatric Nurse or in a similar mental health role.

• Strong clinical assessment and intervention skills.

• Excellent communication and interpersonal skills.

• Ability to work independently and as part of a team.

• Knowledge of relevant legislation and best practices in mental health care.

• A compassionate and empathetic approach to patient care.

• A valid UK driving license and access to a vehicle is desirable but not essential.

Pay: £27.00-£32.00 per hour
