The Colo-Rectal Support Worker will work closely with the colo-rectal clinical team, Cancer Services and Information Services to assist in the coordination of patient care and support.

Under the guidance and supervision of a registered practitioner the Colo-rectal Care Support Worker will facilitate and support patients to regain as normal a life as possible.

Give advice about how to minimise their risk of developing further cancer-related problems.

Give practical and psychological Support to patients undergoing Stoma Surgery.

Give advice about possible signs of recurrence or long-term effects of treatment.

Enable patients to re-access specialist services without delay should they need to do so.

The post holder will be working mainly at hospital environment.

The Colo-rectal Support worker will work independently within cancer standards and operating procedures.

To use own initiative to support Clinical Nurse Specialist (CNS) e.g. wound care, triage fitting of stoma and wound appliances to assist cancer patients throughout the cancer pathway.

To provide support and care to the holistic needs of colo-rectal cancer patients and stoma patients, navigating them through complex health, psychological and social care support and interventions, to ultimately improve the colo-rectal patient experience.

To facilitate, enable and support the multi-professional workforce in the co-ordination of a consistent approach to high quality, evidence based person centred care and support e.g. supporting patients through results, information and signposting.

To contribute to the adjustment or modification of patient individual care plans and disseminate information to the multi-disciplinary team.

Will require fine motor skills for undertaking clinical duties such as venepuncture and the taking of accurate patient observations. Will also require good keyboard skills for entering information into the computerised system.

Be responsible for the communication of condition related information regarding any changes in the patients condition to the appropriate key worker/consultant or member of the Colo-rectal team.

Qualifications

Educated to NVQ level 3 or equivalent or willing to work towards.

Evidence of continuing professional development.

Good knowledge and understanding of health services in Wales.

Good knowledge and experience of Stoma care and Stoma care patients or willing to undertake traaining.

Good knowledge and understanding of colo-rectal cancer and treatments.

Good knowledge and understanding of the emotional, psychological, social and financial impact of cancer diagnosis, treatments and recovery on an individual and their family.

Undertake communication training specifically dealing with difficult situations.

Good knowledge of cancer nursing terminology.

Cancer care qualification/training/experience of working with bowel cancer patients

Knowledge and understanding of cancer nursing services in Cwm Taf Morgannwg

Knowledge of the patient cancer pathway.

Knowledge and experience in care of stoma patients.
