An exciting opportunity has arisen for a Band 5 or Band 6 Community Mental Health Nurse to join Gosport and Fareham OPMH CMHT on NHSP.

Your main responsibilities will include care coordination of service users, Duty Worker on rotation, initial and follow up assessments and supervision and mentorship of junior staff and students.

The Havant and Waterlooville Community Mental Health Team operates from Aerodrome House and is well known for being approachable and open.

The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues. We support our staff in developing their clinical skills in attending post graduate training such as physical health and nurse prescribing.

Due to the nature of this post in order to carry out home visits and attend patient meetings a car driver is essential with access to own car., The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health Team to co-ordinate the care of a complex caseload of service users, supervising Band 5 Community Practitioners and Band 3 Community HCSW’s where appropriate.

Working under the supervision of the Deputy Team Manager/Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer
