Key Responsibilities:
- Responsible for developing close and effective working relationships with key agencies to facilitate a coordinated multi-agency response to the needs of care leavers.
- Work in partnership with young people, their social workers, carers, and other relevant agencies to coordinate, monitor, and review Pathway Plans for young people transitioning to adulthood and independence from public care in Lambeth.
- Support young people in maintaining their accommodation, including preventing rent arrears and repossession of flats by drawing up minimization and engagement plans. Collaborate with benefit agencies and housing officers to assess eligibility and entitlements, ensuring appropriate services are provided.
- Actively support and promote inter-agency pathway planning.
