If you are passionate about providing quality, compassionate care to people in their own homes and you can drive and have access to a car that you can use for work, this is the job for you!
You will be part of a compassionate, forward thinking team working in one of the community nursing hubs. You will work closely with a team of registered community nurses, led by a District Nurse to deliver a wide range of care activities as detailed below.
Core service hours are between 8am and 6pm and you will be required to work weekends on a rota basis. As a flexible employer, we have a variety of shifts covering these hours. We believe in a positive work life balance and are open to discussing hours and patterns of work on an individual basis.
We have positions available in the following community nursing hubs:
- City West
- East Central Hub

Job Description Summary (Main Duties And Responsibilities)
Main duties of the job
- Providing clinical care to patients this includes, but is not limited to: management of wounds and leg ulcers, administration of medicines and catheter care.
- Providing personal care for patients.
- Working with the wider multidisciplinary team.
- Supporting patients to remain at home and avoid hospital admission.
- Promoting self-care and independence.
- To recognise and report changes in the patientโs condition, and any issues in respect of their health and management of care to the registered practitioner.
