Social Worker

What you will do:

• Conduct social care assessments and deliver timely care packages for hospital inpatients prior to their discharge, ensuring a smooth transition.
• Perform mental capacity assessments, make Best Interest decisions, and engage in safeguarding actions as needed.
• Navigate and understand Section 117, actively participating in 117 meetings, ward rounds, and professional meetings.
• Participate in a duty rota to maintain consistent support and service within the team.
• Travel as required and ensure that you are equipped as a car user to reach various locations.
