Responsibilities
- Building relationships with residents in your portfolio, understanding their individual needs and creating support plans that help them live independently
- Helping residents with practical tasks like shopping, medical appointments, and social activities
- Completing yearly reviews to assess financial status and maximise income, arranging aids/adaptations, and care assessments
- Performing essential safety checks including fire alarm testing, telecare alarm testing, health and safety inspections, and water flushing
- Creating opportunities for residents to connect through community activities and events
- Working with other agencies to ensure coordinated support for residents
